Case From - Detail "tag"

Is there any way that I can Add a field on the case form under details tag as to give a description for the selected satisfaction level selected?

Yes you can, both v1.x and v3 supports adding new custom fields to Case object.
Here is a step by step sample in v1.2 (v3.0 coming soon):
